Many families struggle to access services that meet their child’s unique needs. Care Coordination helps ensure no family has to navigate these challenges alone. By identifying needs early and delivering consistent, community-based support, we help reduce reliance on emergency services and prevent unnecessary hospitalizations or out-of-home placements.
With Vibrant, families gain clarity, confidence, and hope. We meet youth and families where they are – offering care in homes, schools, or neighborhood locations convenient to them.
Services include:
-Personalized Care Plans: Collaborative, strengths-based care plans designed to meet the medical, behavioral, and social needs of each child
-Health Coordination: Connections to trusted doctors, therapists, and specialist providers for both short- and long-term care
-Education and Community Resources: Support in navigating schools, assessments, and access to critical resources
-Advocacy and System Navigation: Ongoing support coordinating between providers, agencies, and families to ensure every child’s voice is heard
Eligibility Requirements
- Children & Adolescents
- Under age 21
- Medicaid insurance
- Has Qualifying Conditions (includes medical and behavioral)
- Currently experiencing significant difficulties in areas of their life, such as home, school, medical care, and more.
