All young people benefit from care and assistance as they grow and develop. Often there are times when a period of development can come with challenges that can be especially frustrating and confusing for the young person, as well as all of those around them. It’s during such times that assistance from trained professionals can be of great benefit.
With Care Coordination, a dedicated care manager from our team collaborates with families to find the best way to navigate the complex system of services in New York City. Knowing how challenging it can be, we strive to make things easier for the people who utilize our service by coming to you. We are happy to set our meetings in homes, schools, or other neighborhood locations that may be convenient for the young person and/or family. When we do meet, we begin by identifying all of the young person’s current needs, finding the best providers, then ensuring everyone is working together to reach the outcomes that have been agreed upon by the team, of which the most important parts are the young person and their family. During this time, our team continually finds innovative and relevant ways to be of assistance by staying informed of developing research and information, while always operating from a strength-based and trauma-informed perspective.
Eligibility Requirements
- Children & Adolescents
- Under age 21
- Medicaid insurance
- Has Qualifying Conditions (includes medical and behavioral)
- Currently experiencing significant difficulties in areas of their life, such as home, school, medical care, and more.
